return an item for credit,
repair or advance replacement, download

the form below and fax it to the RMA department
at 541-752-9096. Requests for credit exceeding $2,500
must be
made by phone to
800-469-1676.
We will return the form to you and include
the location to which you should return your product as well
as an
RMA number
for you
to write on your shipping box.
